Non-urgent advice: TYPES OF DATA SHARING

 

 These are the different types of data sharing:

  • Summary Care Record
  • Enhanced Summary Care Record
  • My Care Record
  • Type 1 Opt Out
  • National Opt Out (previously called Type 2 Opt Out)

 

Summary Care Record

A Summary Care Record (SCR) is a national NHS database providing quick access to essential medication and allergy info for urgent care. Our policy is implied consent unless you expressly dissent. If you would like to dissent, please complete and return this national form:

Enhanced Summary Care Record

An Enhanced Summary Care Record (ESCR) is an optional, national extension of basic GP record sharing, containing detailed medical history (conditions, care plans) designed for emergency care. My Care Record is a regional initiative that allows local health/social care staff to view relevant records across different services for direct care. Our policy is implied consent unless you expressly dissent. If you would like to dissent, please complete and return this national form:

My Care Record

In contrast to Enhanced Summary Care Record "My Care Record" refers to a local initiative where health and care staff share a real-time view of records (including test results) for better, coordinated care. 

Bedwell Medical Centre is part of My Care Record, an approach to improving care by joining up health and care information. Health and care professionals from other services will be able to view information from the records we hold about you when it is needed for your care. Our policy is implied consent unless you expressly dissent. There is no separate dissent form, if you dissent to SCR you will also be dissent to MCR.

 

Type 1 Opt Out

If you choose a Type 1 Opt-out, your GP will not share your data for research and planning. However, NHS England will still be able to collect and share data from other healthcare providers, such as hospitals.

To do this you need to fill in an opt-out form and return it to your GP surgery.

Only your GP surgery can process your opt-out form. They will be able to tell you if, and when, you have been opted out.

 

National Opt Out (previously called Type 2 Opt Out)

If you choose this opt-out, NHS England and other health and care organisations will not be able to share any of your personal data with other organisations for research and planning, except in certain situations. For example, when required by law.

If you want to check if you have opted out, you can enter your details again at Make your choice or check your settings in the NHS App. Your GP surgery cannot process this for you.
